 We are an Outpatient Medication Management Practice who care for patients ages 5-65 years of age. If you would like to submit a completed Intake Form along with a copy of the front and back of your insurance card, we will verify if you are In Network with us (Self Pay is an option for those who are Out of Network.) Upon receipt of your information, we will have an intake coordinator contact you within 3 to 5 business days to set up an appointment. 

Our intakes are generally an hour long and are done in person. After that all subsequent appointments are about 20 minutes and Telehealth is an option. If you have any questions we can be reached at 978-606-0011. 

At this time we take the following insurance: Blue Cross Blue Shield, Blue Benefit,Tufts, Aetna, Cigna, United Health Care, Allways, Harvard Pilgrim, and Optum. (Not all plans are covered under the above insurance carriers, please send insurance cards so we can verify coverage.)
